Patient Reference Group

The New Dispensary Patient Participation Group is looking for new members. We are a vibrant, fun group of patients who are dedicated to working with the Practice Team to ensure we deliver the highest possible quality care to everyone registered at the Practice.

The New Dispensary Patient Participation Group Terms of Reference (ToR)

Background:
The Patient Participation Group effective from 31st March 2013.

The purpose of the Patient Participation is to ensure patients are involved in decisions about the range and quality of services provided and, over time, commissioned by their practice.

The Patient Group will be representative of the practice population feeding in its views alongside the findings from surveys and agree with the practice the priority areas for possible change. This will result in an action plan to be agreed between the practice and the PPG.

Document Location:
The document will be retained by the Practice Manager and made available on the practice web site.

Terms of Reference Review:
Annual review from this document agreement date.

Objectives:

  • Build a relationship between the Practice and its patients.
  • Develop a structure that gains the views of patients and enables the practice to obtain feedback from the practice population.
  • Establish a PPG comprising only of registered patients and using its best endeavours to ensure the group is representative of its registered patients.
  • Collate patient views using survey’s etc.
  • Agree areas of priority within the PPG.
  • Provide the PPG with an opportunity to discuss survey findings and reach agreement with the Practice on possible changes to services.
  • Agree action plans within the PPG and seek Practice agreement to implementing changes.
  • Liaise with other patient Participation Groups in the area.
  • Liaise with the NHS South Warwickshire Clinical Commissioning Group.

Deliverables:
Publicise actions taken and subsequent achievements.

Reporting Mechanism:
Minutes will be retained by the PPG Chair, Practice Manager and made available on the practice web site.

Reporting PPG activities will be regularly fed back to all patients by newsletter, poster display, practice web site or similar format. The Practice Manager to feed back to practice staff and partners. This will enable everyone to be aware of the activities of the group and may increase involvement and interest.

PPG Membership:
A group of circa 10-person representative of the practice population and representatives from the practice. This ideally means clinical as well as managerial staff.

Chair – For continuity, the chair is better represented by a nominated individual. If a volunteer agrees to this position a deputy must be identified. The position of Chair to be reviewed annually. If no volunteer can be identified, the chair task will be rotated throughout the group. The period between rotations to be agreed within the group.

Secretary - For continuity, the secretary is better represented by a nominated individual. If a volunteer agrees to this position a deputy must be identified. The position of Secretary to be reviewed annually. If no volunteer can be identified, the secretary task will be rotated throughout the group. The period between rotations to be agreed within the group.

Quorum – Four members will represent a quorum one of which must be a representative from the practice.

PPG meeting frequency to be based on an eight-week period. Where necessary additional meetings will be convened.
