SWGP Federation

South Warwickshire GP Patient Forum and CEPN:
We joined forces late last year with thirty-four other GP practices across south Warwickshire to form a GP Federation and improve the services we can off to our patients. We’ve been working hard behind the scenes to bring you even better care such as the Over 75s health checks. These deliver proactive health reviews for people over the age of 75. Under the scheme, we will be contacting patients aged 75 and over to arrange detailed health assessments, with a view to identifying proactive measures to help maintain good health.

What is a GP federation?
A federation is a group of GP practices working together to provide health services to patients. We will share back office resources such as training and purchasing and enables us to bid for funding to provide additional services for our communities.

With the combination of a growing population and people living longer with more complex health conditions, GP practices need to find better ways of working so that we can continue to provide you with high quality care.

How will this affect me?
All our patients will continue to access services in the normal way. The New Dispensary will continue to operate as an independent practice and you will still see the same GPs and practice staff. One of the main reasons for setting up the federation was to enable us to bid for funding to provide additional services within our local area.

Community Education Provider Network (CEPN)
A CEPN is a group of practices that come together to provide and deliver training opportunities in its locality to ensure there is a well trained workforce for years to come. Current national workforce scoping suggests we will have a shortage of trained GPs and Practice Nurses in the next ten years as the numbers qualifying are not high enough to meet those retiring. SWGP have been engaging with Health Education West Midlands (HEWM) and other stakeholders to learn more about these networks which have been successfully developed in other areas of the country.

HEWM are shortly inviting applications from federations or similar organisations to bid for a small amount of funding over the next few years to develop a CEPN in their own locality. This application should focus on practice and community nursing provision. SWGP intend to submit a bid in September to benefit both its practices and patients. If successful we would focus on scoping our current nursing workforce to understand our position whilst up skilling our practice nurses so they can mentor student nurses.

Patient Forum
After the popularity of our presentation in June to the CCGs patient forum we agreed to follow this with another meeting at a larger venue. This will be held on Thursday 17th September 19:00 – 20:00 at a venue to be confirmed dependant on numbers. Will we present our current progress, forward view and answer your questions. The New Dispensary joins South Warwickshire GP

The New Dispensary has joined forces with thirty-four other GP practices across south Warwickshire to improve the services we can off to you, our patients. Although you may not immediately notice any changes, we’ll be working behind the scenes to bring you even better care.

What is a GP federation?
A federation is a group of GP practices working together to provide health services to patients and share back office resources such as training and purchasing.

With the combination of a growing population and people living longer with more complex health conditions, GP practices need to find better ways of working so that we can continue to provide you with high quality care.

We have set up South Warwickshire GP (SWGP) to enable us to bid for funding to provide additional services for our communities and reduce the amount of time spent on administration.

How will this affect me?
All our patients will continue to access services in the normal way. The New Dispensary will continue to operate as an independent practice and you will still see the same GPs and practice staff.

Most changes will affect ‘back office’ functions where there are opportunities to work together to do things more efficiently.

Benefits for patients
One of the main reasons for setting up the federation was to enable us to bid for funding to provide additional services within our local area.

SWGP has already successfully secured funding from South Warwickshire Clinical Commissioning Group to deliver proactive health reviews for people over the age of 75. Under the scheme, we will be contacting patients aged 75 and over to arrange detailed health assessments, with a view to identifying proactive measures to help maintain good health.

Working with our new care co-ordinator, those patients who are frail or in poorer health will be offered further support will in partnership with Age UK Warwickshire, which will cover health and social care needs. This will include helping patients access other services available to them.

One GP from each practice acts as a shareholder and nominally holds the shares for their partnership. In our case, our shareholder is Dr Nilesh Vara They will take an active role in directing the priorities of the federation and ensuring they meet the needs of our patients.
