Ordering Repeat Prescriptions online
The online form you may have been using to request your repeat medication via our website will cease from 1st May 2018.
We aim to make this process much easier for both yourself and the practice by using patient access via this link https://patient.emisaccess.co.uk You will need to register with the practice for on-line access if you have not already done so, all the necessary forms are available on the Online Services page of this website. Please bring the form with identification into the practice to enable our reception staff to supply you with your login details between the hours of 10.00am and 5.00pm.
Once this has been done the process will be simple by registering on the patient access website. You will then see your medication on the repeat medication screen and all you have to do is tick the medication you require. This will go directly into our clinical software for the Drs to check and sign.
If your request is rejected you can see the reason why by logging into the patient access website and the reason will be on your medication screen.
Please apply for patient access if you wish to continue to order your repeat medication on line. This current online form for repeat medication will cease on 1st May 2018. Thank you.
We are now introducing an online booking service for appointments and repeat prescriptions. You must be over 16 years of age.
To register we will need your email address to be sent to email@example.com. After 48 hours please provide proof of identification (driving licence/passport) to reception to obtain your registration details. This will provide information on how to register with www.patient.co.uk. Do we have your current telephone/mobile number also? Confirm at reception.
You can now access your medical record online, if you wish to have access please download the information and forms below, complete and bring to reception. Please remember to bring some form of photo identification with you, this will also be needed on collection of your registration password. If you have already got access to online repeat prescriptions and appointments and have a passcode, we still require the forms to be completed with your consent to enable us to activate your record. Thank you.
